What you’ll be doing

Clinical Responsibilities:

Provide comprehensive psychosocial assessments.
Assure responsibility for the development & implementation consumer’s Individual Plan of Service.
Act as an advocate and/or a liaison coordinating services among providers of service and referral source.
Maintain consumer records – meeting all appropriate standards according to HNBHS and all regulatory bodies.
Participate in legal proceedings including petitioning and testifying in clinical emergencies.
Facilitate the coordination of consumer intakes, discharges, and transfers to and from other facilities programs, agencies.
Facilitate communication with guardians, payees, and family members regarding consumer care, financial and treatment issues.
Meet with assigned consumers face-to-face
Link consumers with community organization and community integration.

Anticipated Work Schedule

Regular Hours: Monday-Friday, 9:00am-5:00pm (flexible schedule available)
On-Call Rotation (varies by location):
One day per week every other week
One weekend every other month

On Call Compensation Rates:

$50 additional for on-call duty Monday to Friday
$100 additional for on-call duty on Saturdays and Sundays
$150 additional for on-call duty on holidays

Job Requirements

Required: Bachelor’s degree in Social Work (BSW) with current licensure as a Licensed Bachelor Social Worker (LBSW) or Limited Licensed Bachelor Social Worker (LLBSW).
Preferred: Master’s degree in Social Work (MSW) with licensure as a Licensed Master Social Worker (LMSW) or Limited Licensed Master Social Worker (LLMSW). Licensure as a Licensed Professional Counselor (LPC) or Limited Licensed Professional Counselor (LLPC) is also preferred.
Alternative: Bachelor’s degree in a Human Services field is acceptable if accompanied by a Social Service Technician (SST) certificate.
Experience: Prior case management experience in a mental health or rehabilitation setting is preferred.
